Cathedral of St. John, Warsaw, Poland
The Cathedral of St. John began as a parish church in the 15th century, being designated a cathedral in 1798.  Of course it was also pretty much destroyed in World War II, and when rebuilt, this façade was apparently redesigned in a Mazovian Gothic style.  It is prominently visible in the Old Town.
